Below are the required permission forms for each session type. These must be completed before your child’s first session. Families may either print and fill out a hard copy to bring in, or scan and email the completed forms. (If you need us to print these for you, please give us a call). Hub Sessions also offer the option to complete the form electronically via Google Forms—simply submit online, with no printing required.
